Explorer S-45 was a NASA ionosphere beacon satellite meant to map the shape of the ionosphere from a highly elliptical orbit reaching about 181,000 km. The Juno II launch vehicle malfunctioned after second-stage separation and the upper stages failed to ignite, so the satellite never reached orbit.
